In this image, it is apparent that the character in this picture has undergone or is about to undergo a medical surgery of some kind. We know this because of the tape on his eyes and the tube of what can be assumed is anaesthetic in his mouth. From this picture we know that the character shown is definitely a significant as on the theatrical poster below, we see a medical arrangement and doctors standing around an operating table. From that we know that the film is based around some kind of surgery or event in a hospital. What is strange in this picture however, is the fact that the character appears to be crying. This creates a sense of fear and mystery for the audience as of course when under anaesthetic during a surgery, you are completely unconscious and so there for the tear is something that people will question.
Tears are associated with sadness and especially in this situation it creates a dark and fearful image which of course fits in with the film's genre, Thriller.
